Guide-quality immune system: hold the narrative bar + kill reference-staleness durably (epic #14310)

Where I align

The proposal also correctly reuses existing primitives instead of inventing a parallel docs governance system.

If Prong 3 says "extend GapInferenceEngine" too broadly, it will either overload capabilityGap strings with linter results or force graph code to parse markdown. That is the wrong boundary.

Challenge 2: generated reference needs a strict source boundary
OQ2 should not resolve as "generate reference" globally. The line should be source-owned and mechanically derivable:

For OQ4, I would not wait for the full immune-system epic before closing the #14340-class hole. I would graduate one standalone first-win ticket for ai:lint-guides, then let the broader immune system graduate as an epic after OQ2/OQ3 converge.
