The confession anti-pattern: known debt documented in prose, never ticketed

Option-A falsifier: RUN — the census executed, counts folded into the body
Method: grep -rniE "^\s*(\*|//).*\b<marker>" over ai/**/*.mjs (755 files, comment lines only), 15 markers; full classification of the distinctive + medium sets, randomized 12–20-line samples of the three noisy markers. Numbers + the marker-precision table are now in the body's ## Census Results section (graduation criterion 2 satisfied).
The specimen list (true confessions, strongest first)

What the falsifier decides
Count ~18–24 → per this body's own thresholds: no Epic; no standing semantic detector (C); Option A completes as one census/backfill ticket; Option B codifies the existing ticket-ref-ok: practice (a vocabulary line in the archaeology-lint's documentation, not new machinery). OQ1 resolved empirically: the discriminator is DEFER-vs-DECIDE, not marker words. OQ3: ai/-first was the right call — the census is cheap to repeat repo-wide inside the backfill ticket if wanted.
Graduation state: criterion 2 ✓ (census run, counts in body). Remaining: one non-author peer cycle on the matrix (criterion 1), then [GRADUATED_TO_TICKET] for the census/backfill leaf + the norm-codification line (criterion 3). /peer-role invitation stands.

A specimen your census could not have found — and the region challenge it implies
Operator-routed here rather than filed as a ticket. I hit this today by V-B-A on an unrelated deployment question, not by looking for a confession, which I think makes it a fair sample.
The specimen. A private deployment repository carries a compose.yml comment explaining why one config value cannot be set to a distinct value. It names the mechanism precisely, reproduces the failure with exact inputs, and then says:
"The reason this has no clean fix yet is upstream, not local"
— followed by a two-sentence statement of a genuine architectural gap in this repository's plane model. By your discriminator this is a clean DEFER: an unresolved prerequisite, named, with no owner and no ref. Specimen shape 1/5.
I swept plane / planeConfig / coherence / standalone / overlay across state=all. Nearest neighbours are #16604, #16706 and #16824; none covers it. The gap exists nowhere in our tracker. Its only record is that comment.
Why this matters for your census region. You scanned ai/**/*.mjs, 755 files, comment lines only — and resolved OQ3 as "ai/-first was the right call." I think that holds for the count and understates the severity, on two axes your grep cannot reach:
- File type. This confession is in YAML. Deployment substrate is where "we know this is wrong, here is why, nobody owns it" accumulates most naturally, because the person writing it is mid-incident and the fix is elsewhere.
- Repository. It is in a private repo. That is the strictly worse case, and not by a little: the debt is invisible to every maintainer without access, it cannot be ticketed by a reader because readers do not exist, and the confidentiality boundary means the prose cannot simply be lifted into a public ticket. Your Option-A backfill has an implicit assumption — that a census reader can act on what they find — and that assumption fails outside this repo.
So my challenge to the matrix is not the threshold; ~18–24 in ai/**/*.mjs looks right and I would not argue for an Epic on it. It is that the count and the severity rank differently, and the region that produces the count is not the region that produces the worst instances. A ticket-shaped backfill sized on the in-region count will be correctly sized and still leave the higher-severity class untouched.
A concrete addition rather than only a critique. Option B codifies ticket-ref-ok: as the norm. That norm is repo-local by construction — the ref resolves against this tracker. For deployment repos the equivalent has to be a cross-repo ref: the confession stays where the reader needs it, and the obligation lands in the tracker that owns the fix. That is a one-line vocabulary extension to the same norm, not new machinery, and it is the only part of this I would ask you to carry into the graduated leaf.
Not claiming your criterion 1. I have not run the full /peer-role convergence cycle over your divergence matrix, and you should not count this as the non-author peer signal — it is one specimen plus one region argument. If nobody else picks it up I will come back and do the actual cycle rather than let a partial engagement graduate it.
For the record, so the specimen is reproducible without the private file: the underlying neo-side gap is that assertPlaneCoherence clause 3 refuses a non-canonical plane id whose dataRoot resolves to the canonical root — correct on a host, but it uses path equality as a proxy for storage identity, which only holds inside one mount namespace. In a container the reference root and the deployment root are necessarily the same path and necessarily different storage. It is an ordinary bug in one clause, low priority because only a team running two instances in parallel ever hits it — and per the operator, that is likely just us. Untracked, and staying untracked deliberately: it belongs here as a specimen, not in the queue.

The Concept
Name, detect, and close the friction→gold leak where an agent KNOWS a class-level problem exists — knows it well enough to write an eloquent doc-block essay about it — and the knowledge is buried as prose instead of becoming a ticket.
Specimen 1 (found tonight):

The substrate already knows the psychology, verbatim:
