Intent

Implement Heartbeat-Bypass Detection for push-capable identities, per ADR 0002.

Acceptance Criteria

Update swarm-heartbeat.sh to consult the WAKE_SUBSCRIPTION graph state on startup and per-cycle.
Skip heartbeat polling for identities with harnessTarget IN (mcp-notifications, a2a-webhook).
Retain heartbeat polling as fallback for bridge-daemon, disabled, and none harness targets.
Build against the contract defined in learn/agentos/decisions/0002-phase3-wake-substrate-standards-alignment.md §6.5.
